The ECR Form is used to communicate employee status changes to Human Resources. It should be completed when the following occur:

  • Changes to job status (hours/schedule/pay)
  • Job promotions, demotions or transfers

  • Termination (with prior approval from Human Resources)

To be completed after first six (6) months of employment for professional/ administrative employees and the first four (4) months for office/clerical and service employees.
